  • Purpose
    During the coronavirus disease 2019 (COVID-19) pandemic, several studies reported changes in the distribution of microorganisms that cause major legal, respiratory, and gastrointestinal infectious diseases and increases in the antimicrobial resistance rates in Korea. On the other hand, there has been little domestic research on the causative organism of urinary tract infection (UTI). This study investigated the influence of the COVID-19 pandemic on the distribution of causative organisms and the antimicrobial resistance rate in UTI.
  • Materials and Methods
    This study analyzed 17,201 urine cultures retrospectively from patients who visited Chung-Ang University Hospital from January 2018 to December 2021. Tests were then conducted to determine if there was a significant difference between the data for the eight quarters of the pre-COVID-19 period (January 2018 to December 2019) and the data for the eight quarters post-COVID-19 period (January 2020 to December 2021).
  • Results
    Escherichia coli was the most common causative organism in all periods, but it decreased in the post-COVID-19 period. Enterococcus faecalis increased in the post-COVID-19 period. The ciprofloxacin resistance rate of E. coli and Klebsiella pneumoniae increased, but the ciprofloxacin and levofloxacin resistance rate of E. faecalis decreased.
  • Conclusions
    There was little difference in the causative organism distribution of UTI and antimicrobial resistance rates before and after the COVID-19 pandemic. On the other hand, changes in some causative organisms are identified. Nevertheless, because this study was limited to a single medical institute, data from a broader spectrum of bacterial species collected from multiple institutions will be needed to obtain definitive results.

2019년 중국 우한에서 coronavirus disease 2019 (COVID-19) 첫 환자가 보고되었고 국내에서는 2020년 1월 첫 환자가 발견된 이후 지난 2년여 동안 전 세계적인 COVID-19의 대유행이 있었다. 고강도의 사회적 거리 두기 및 마스크 의무 착용 등 사회적인 방역 지침이 강제화되었으며 의료기관 내에서도 감염병 관리의 지침 변화 등 적극적인 방역 정책이 시행되었다. 이는 COVID-19의 확산을 막는 데도 크게 기여하였지만, 의료기관이나 지역사회에서 COVID-19가 아닌 다른 감염 질환의 발생에도 영향을 준 것으로 추정된다. 질병관리청의 2020년 법정 감염병 발생 현황과 호흡기 전파 감염병 현황에 대한 보고에 따르면, 2020년의 법정 감염병 신고 건수가 전년도에 비해 46.6%가 감소하였으며 COVID-19와 비슷한 전파 경로를 갖는 호흡기 전파 감염병의 발생 건수도 51% 감소하였다[1]. 국내 호흡기 전파 바이러스 질환의 유행 양상을 비교한 또 다른 연구에서도 인플루엔자를 비롯한 호흡기 바이러스 관련 감염병이 COVID-19 이후 현저히 감소한 사실을 알 수 있었으며[2], 소화기 감염병의 추세를 보고한 논문에서도 바이러스성 감염의 발생률이 31.9% 감소하는 등의 변화된 양상을 확인할 수 있었다[3].
요로 감염은 전 연령에서 발생할 수 있는 가장 흔한 비뇨기과 질환 중 하나로[4], Escherichia coli가 원인 균종 중 가장 높은 비율을 차지하며 경험적 항생제로 ciprofloxacin 및 2, 3세대 세파 계열 항생제가 권고되고 있다[5]. COVID-19 이후 요로 감염을 제외한 다른 감염 질환들의 원인 균종 분포 비율 변화가 보고되고 있으며, COVID-19 환자의 치료를 위한 항생제 사용의 증가가 내성률 변화에 많은 영향을 끼칠 것으로 추정되지만, 요로 감염에서 COVID-19 이후로 원인 균종의 분포 비율이나 항생제 내성률에 변화가 있는지에 대한 연구는 많지 않았다. 해외에서 2개 의료기관의 여성 환자 소변 검체를 대상으로 한 연구가 보고된 바 있으나[6], 국내에서 비슷한 사례의 연구는 거의 없는 것으로 확인되었다. 이에 저자들은 COVID-19 이후 요로 감염에서도 이러한 원인 균종의 분포나 항생제 내성률에 변화가 있는지를 조사함으로써 요로 감염의 빠르고 효율적인 치료 및 관리에 도움을 주고자 하였다.
COVID-19가 시작된 시점을 기준으로 전후 2년인 2018년 1월부터 2021년 12월까지 4년간 중앙대학교병원에 내원한 환자의 소변 배양 검사 중 의미 있는 원인균이 동정된 17,201건을 대상으로 연구를 진행하였다. 동정된 원인 균종 및 각 균종별 항생제의 내성률 검사 결과를 후향적으로 분석하였다.
적절하게 채취한 요는 배양에 앞서 원심분리 및 침전물을 도말하여 그람 염색으로 요중 세균 및 백혈구를 확인하였고, 이후에 혈액 한천 배지와 MacConkey 한천 배지에 0.001 ml씩 접종하여 37℃에서 18-24시간 배양하여 요 1 ml 중 세균 집락 수를 산출하였다. 세균 동정과 항생제 감수성 검사는 Vitek MS (Biomërieux, Vitek Inc., Hazelwood, MO, USA)와 Vitek II (Biomërieux, Vitek Inc.)를 이용하였다[7].
COVID-19 유행 이전 시기를 2018년 1월부터 2019년 12월까지로, 유행 이후 시기를 2020년 1월부터 2021년 12월까지로 정의하였다. 모든 데이터를 3개월씩 총 16분기로 나눠서 분석하였으며, COVID-19 이전과 이후 각각 8분기 데이터들을 Mann–Whitney 검정방식으로 검정하였다. 통계 프로그램은 IBM SPSS version 26.0 (IBM Corp., Armonk, NY, USA)을 사용하였으며 p-value<0.05를 통계학적으로 유의미한 차이가 있는 것으로 정의하였다.
2018년 1월부터 2021년 12월까지 COVID-19 전후 2년간 모든 시기에서 E. coli의 분포 비율이 43-54%로 요로 감염 원인균의 가장 높은 비율을 차지하였으며, 그 뒤를 이어 그람 음성 세균인 Klebsiella pneumoniae가 9-14%, 그람 양성 세균인 Enterococcus faecalis 및 Enterococcus faecium이 각각 5-14%, 6-11%의 비율을 차지하였다(Table 1). 그 밖에 Pseudomonas aeruginosa가 3-9%, Acinetobacter baumannii 및 Staphylococcus aureus가 1-3%의 분포 비율을 보였다(Table 1). 대부분 균종들은 COVID-19 이전 시기인 2018년 1분기부터 2019년 4분기의 8분기 분포 비율과 COVID-19 이후 시기인 2020년 1분기부터 2021년 4분기의 8분기 분포 비율에서 유의미한 차이를 보이지 않았으나, E. coli는 COVID-19 이전에 비해 COVID-19 이후에 그 분포 비율이 유의미하게(p<0.001) 감소한 것으로 나타났다(Fig. 1). 한편 E. faecalis는 COVID-19 이전보다 이후에 분포 비율이 유의미하게(p<0.001) 증가하여(Fig. 2), COVID-19 이전에는 K. pneumoniae에 이어 3번째의 분포 비율을 보였으나 COVID-19 이후에는 K. pneumoniae와 비슷한 비율을 차지하는 것으로 나타났다.
주요 균종들의 항생제에 대한 내성률도 2018년 1분기부터 2021년 4분기까지 총 16분기에 걸쳐 분석하였다(Table 2). 가장 흔한 균종인 E. coli는 ampicillin에서 68-76%의 내성률을 보였고 cefotaxime과 ceftazidime은 31-40%, cipro-floxacin은 46-57%를 나타내었다. K. pneumoniae는 cefotaxime과 ceftazidime에서 36-64%, cipro-floxacin에서 36-61%의 내성률을 보였고, carbapenem계열 항생제들에서도 0-25%의 내성률을 보였다. 그람 양성 세균인 Enterococcus를 살펴보면, E. faecalis는 ampi-cillin과 vancomycin에는 거의 내성을 보이지 않았으나 ciprofloxacin에는 14-36%, gentamycin과 strepto-mycin에는 각각 41-61%, 10-24%의 내성률을 나타내었고, E. faecium은 ampicillin에는 94-100%, vancomycin에 49-78%의 내성률을 보여 E. faecalis와는 대조적이었으며 linezolid에는 내성이 거의 없었으나 quinupristin/dalfo-pristin은 0-32%의 다양한 내성률을 보였다. COVID-19 이전과 이후 시기를 비교하였을 때 대부분의 균종은 항생제 내성률에서 큰 차이를 보이지는 않았으나 일부 균종의 일부 항생제 내성률은 의미 있는 수준에서의 변화가 관찰되었다. E. coli의 ciprofloxacin 내성률을 살펴보면, COVID-19 이전 8분기 내성률에 비해 이후 8분기의 내성률이 유의하게(p=0.001) 증가되었고(Fig. 3), K. pneumoniae의 cipro-floxacin 내성률 또한 COVID-19 이전 8분기에 비해 이후 8분기에서 유의하게(p=0.007) 증가되어 있는 것을 확인하였다(Fig. 4). E. faecalis에서는 fluoroquinolone계열 항생제인 ciprofloxacin과 levofloxacin의 COVID-19 이후 항생제 내성률이 이전에 비해 유의한 수준으로(p=0.02) 감소되었다(Fig. 5).
요로 감염은 가장 흔한 비뇨기과 질환 중 하나로 성별과 무관하게 전 연령층에서 발생할 수 있으며, 특히 여성의 경우 약 절반에서 일생에 한 번은 요로 감염을 경험하는 것으로 알려져 있다[4]. 요로 감염을 일으키는 주요 원인균으로는 장내세균이 있으며 그중에서도 E. coli가 가장 높은 빈도를 차지한다는 것이 여러 연구와 보고들로 잘 알려져 있다[4,8,9]. E. coli를 제외한 다른 원인 균종들의 분포 비율은 시간이 지나면서 변해왔는데, 과거에 비해 Pseudomonas, Klebsiella 등의 그람 음성 세균과 Enterococcus 등의 그람 양성 세균의 비율이 점점 늘어나는 추세이다[4,9,10].
요로 감염의 치료에 있어서 적절한 경험적 항생제의 선택은 중요하며, 따라서 각 원인 균종별로 항생제의 내성률에 대한 연구와 보고가 지속적으로 이루어져 왔다. 2020년 국가 항생제 내성균 조사 결과에 의하면[11], E. coli의 경우 경험적 항생제로 많이 사용되는 ciprofloxacin의 내성률이 44.8%로 이전과 비슷한 수준이었고 cefotaxim과 ceftazidime도 각각 35.2%, 10.0%로 이전과 비슷한 수준으로 유지되었다. Carbapenem계열 항생제들의 내성률은 1% 미만이었다. 최근 요로 감염 원인균으로서 점차 비율이 증가하고 있는 K. pneumoniae의 경우, ciprofloxacin의 내성률은 33.2%로 이전과 비슷하였으며, cefotaxime과 ceftazidime의 내성률도 각각 38.9%, 26.5%로 역시 이전과 비슷한 양상이었다. Carbapenem계열 항생제들의 내성률은 2%대였다. 요로 감염 원인균 중 그람 양성 세균으로 점점 비율이 증가하고 있는 Enterococcus 중에서 E. faecalis의 항생제 내성률은, 비교적 감수성이 있다고 알려진 ampicillin의 내성률이 2020년 8.3%로 보고되어 증가하는 양상이었고 ciprofloxacin 내성률은 31.4%로 감소를 보이고 있는 반면, vancomycin 내성 균주는 드물지만 꾸준히 분리되고 있었다. 항생제 내성률은 시간이 지남에 따라 계속 변화하기 때문에 현재의 요로 감염 원인 균종 분포와 균종별 항생제 내성률을 잘 파악하는 것이 요로 감염의 효율적인 치료에 있어서 중요하다고 할 수 있다.
한편, 2020년부터 시작된 COVID-19의 대유행으로 마스크 착용 의무화, 사회적 거리 두기 등의 강력한 방역조치와 더불어 의료기관 내 감염병 관리의 지침 변화 등으로 인해 여러 감염질환의 발생에 변화가 생긴 것으로 추정된다. 즉, 법정 감염병 신고 건수 및 호흡기 전파 감염병의 발생 건수와 바이러스에 의한 소화기 감염병의 발생률이 감소하는 등 변화된 양상을 확인할 수 있었다. 하지만 요로 감염에 대해서는 비슷한 방식으로 비교를 한 연구가 없었기에, 본 연구를 통하여 국내에서 COVID-19 유행 전후로 요로 감염의 원인 균종과 항생제 내성률에 변화가 있었는지를 알아보고자 하였으며 몇몇 균종들에서 유의미한 변화 양상을 확인할 수 있었다. 우선 원인 균종 분포의 변화에 있어서는 COVID-19 유행 이전에 비해 E. coli는 감소하였고 E. faecalis는 증가하였으며, 항생제 내성률 변화에서는 E. coli 및 K. pneumoniae의 ciprofloxacin 내성률은 증가하였고, E. faecalis의 flu-oroqu-inolone 내성률은 감소하였다. 그러나 그 밖에 다른 대부분의 원인 균종들에서는 분포 비율의 변화나 항생제 내성률의 변화가 COVID-19 전후로 뚜렷하게 관찰되지는 않았다. 요로 감염에 대해서는 COVID-19의 유행이 주요 법정 감염병 및 호흡기나 소화기 감염병에서만큼 많은 영향을 주지 않는 것으로 분석되었으며, 변화가 관찰된 특정 균종들에 대해서는 이러한 변화가 다른 의료기관에서도 비슷한 패턴으로 나타날지, 또한 그 원인은 무엇일지에 대해 좀 더 많은 연구가 필요할 것으로 생각한다. 다만, E. faecalis의 분포 비율 증가에 대해서는 해외의 다른 연구들을 통해 비슷한 경향을 발견할 수 있었다. COVID-19로 중환자실에 입원한 환자의 검체에서 나온 균종을 분석하여 Enterococcus spp.가 원인 균종 중 가장 높은 비율을 차지하였으며, 동시에 COVID-19가 아닌 질환으로 중환자실에 입원한 환자와 비교하였을 때 Enterococcus spp.가 유의미하게 증가되었음을 보고하기도 하였고[12], 이와 비슷하게 COVID-19로 입원한 환자의 원인 균종을 분석 후 대조군과 비교하여 Enterococcus spp.가 유의미하게 증가되어 있음을 확인한 연구도 있었다[13]. COVID-19로 입원한 요로 감염 환자의 소변 검체에서 Enterococcus spp.가 가장 높은 비율을 차지하였으며, COVID-19 이전인 2019년에 같은 의료기관에 요로 감염으로 입원한 환자의 원인 균종 분포 비율과 비교하여 Enterococcus spp.가 유의미하게 더 높은 비율을 보이고 있음을 관찰한 연구도 있었다[14]. Enterococcus와 COVID-19와의 연관성을 보여주는 50여 편의 논문을 분석하여 COVID-19 이후로 Enterococcus spp.의 증가가 위협적인 수준이 되었으며, 그 원인은 아직 밝혀지지 않았으나 COVID-19의 감염으로 장내 미생물 군집 환경이 Enterococcus spp.에 친화적으로 바뀐 뒤 장내 투과성의 증가로 인해 다른 장기로 침투하였을 가설을 제시한 논문도 있었다[15]. 이렇듯 아직 구체적인 원인은 밝혀지지 않았으나 Enterococcus spp.가 COVID-19 이후로 증가세에 있다는 연구 결과는 많이 발표되었으며, 본 연구에서의 E. faecalis 증가도 이러한 경향을 반영하는 것으로 볼 수 있다.
본 연구는 몇 가지 한계점을 가지고 있다. 첫째, 17,201건의 소변 배양 검사 결과에 대해 각각의 환자가 가지고 있는 다양한 요소들을 고려하지 않았다는 점이다. 즉, 나이나 성별, 기저 질환 등과 같은 요소들을 분리하여 분석을 진행하지 못하였다. 특히 입원환자와 외래환자를 구분하여 분석하지 못해 지역사회 요로 감염과 의료기관 내 요로 감염에서 COVID-19의 영향력이 어느 정도인지를 판단할 수가 없었다. 둘째, 국내 의료기관 전체를 대상으로 한 것이 아닌, 단일 의료기관의 자료를 바탕으로 분석하였기 때문에 대표성이 떨어진다고 할 수 있다. 마지막으로 균종 분포 비율 변화와 항생제 내성률 변화만을 분석하였고, 유의한 결과를 보인 이유에 대한 가설이나 증거를 제시하지 못하였다. 이러한 한계점들을 향후에 좀 더 보완하여 후속 연구를 진행한다면, COVID-19 이후 요로 감염의 진단 및 치료에 도움이 될 수 있을 것으로 기대해 본다.
COVID-19 유행이 단일 의료기관의 요로 감염 원인 균종 분포 및 항생제 내성률 변화에 많은 영향을 주지는 않은 것으로 분석되었지만, 몇몇 균종들의 분포에서 유의미한 수준의 변화를 보였으며 항생제 내성률 변화에서도 일부 균종의 일부 약제에 대한 변화 추세를 확인할 수 있었다. 앞으로 더 많은 의료기관의 소변 검체를 대상으로 COVID-19 유행 전후의 균종 분포 및 항생제 내성률 추이를 분석할 필요가 있을 것으로 생각한다.
